예약하다 means “to reserve.” 예약했어요 is the polite past form and means “I made a reservation,” which is the natural way to say you have one.
At a restaurant or hotel, add your name: 민지 이름으로 예약했어요 means “I reserved under the name Minji.”
The past tense is correct because the reservation was made earlier, even though it is relevant now.
Staff may ask 성함이 어떻게 되세요? for your name, or 예약 번호가 있어요? for a confirmation number.
